A geothermal unit uses the ground to either gain heat for the house (winter) or get rid of heat (summer). There are different ways to this, some being a closed loop where water is circulated in an underground loop and back to the unit or an open loop where ground water is taken in, the heat is added or removed and then drained away.

As stated, it works similar to an air conditioner, but instead of trying to push heat into 95 degree air in the summer with your outdoor AC unit, you push the heat into the ground which is at 50 -60. The water running through the ground replaces the radiator of your air conditioner.

To get air coming out of the furnace that is warmer than the water going in, a compressor and refrigerant is necessary, working just like a fridge with warm coils on the back (the fan blows through these coils to warm the air in your furnace). The refrigerant exchanges heat with the water that runs through the ground.
