| most dam failures are due to inadequate spillway design, either not large enough to handle excess flow or just not designed period. Other causes of failure are cracks that lead to erosion under or through the dam. can get tremendous pressures at base of dam to cause uplift. All it takes for failure is a little tremor from an earthquake. God forbid any such coincidence. usually try to drawdown the reservoir as fast as possible thus leading to flooding downstream.
