On every o care policy I have seen trips by plane or copter arent covered ---------------on premium policies it isn't covered. If you have a situation such as a at risk pregnancy the DR will advise you to buy the membership since they will then accept ambulance ground rates like the other poster said. . A good possibility in this case will be a advised medical bankruptcy
