http://www.mikesheating.com/ We bought our system from Mike's in 2005. They are a very large volume dealer and ship anywhere.The concrete crew will likely use grade stakes to level the fill. If so you will need to use a hole saw to cut a hole for each stake. We used 3/4" foam. In ND I believe the heat loss will be much greater through your doors than the floor. I would use 3/4" even if you want to go thicker. Unless your fill is perfectly level the 2" will not sag into the little depressions you have in the fill. Neighbopr used 2 layers of 3/4". The cost per inch of insulation is the same whether you use 3/4" or 2". We put our rebar on 1 foot centers and zip tied the 1/2" to the rebar. We have had no issues with leaks in 6 years. I would staple it to the foam if I did it again.
