Mine, poured last year, was vapor barrier, 2 inch 250 pink board, staple the 5/8 tubes to the foam, 5/8 rebar on 14 inch centers.

I wouldn't use 5/8 tubing again though. I was going to use 1/2, my uncle thought 3/4, so we compromised. I would just cave in to the 3/4 next time. It is a lot easier to find fittings for 3/4.

Engineering specs I've read before on rebar insisted on the need for 1 inch clear concrete cover on the bar for strength. Attaching the tubing to the rebar sounded like a way to compromise the reason you are putting the rebar in for in the first place.

I know many others though attach to the rebar, including a big commercial outfit in our area.
