What I've done a couple of times is written a note on the bottom on the ticket explaining why I left no tip. Then they know.
My brother and SIL own a couple steakhouses in western ND. Some people think there job is to only collect tips. They may start off being good servers, but it can go down hill. I've seen it many times.
