We only drink water when we eat out and very seldom have a need for a refill, however, if the waitress doesn't at least come back by the table and "check" on us, she's not doing her job and I tip accordingly. My normal tip is 25% up. I think the servers in the lower priced cafes work just as hard as the servers in the high priced restaurant's and I try to tip them all the same.
