You're not going to become diabetic with a naturally occurring low blood sugar. Many people have normally low fasting blood sugars (I've seen mine in the 70's too). But hypoglycemia can be brought on by a number of reasons. Some may be of no consequence and others may be, especially if someone has symptoms. The fact that someone has fasted for a period of time and then had a low blood sugar would cause me to wonder what it had been before. Was it normally low or did it go low because of eating? No way of knowing without additional testing. If he has access to a glucose meter he can easily check this out. Many people who are overweight or obese also produce large amounts of insulin- in the short term their blood sugars are okay (fasting or Hgb A1C)- in the long-term maybe not so much. Now would be the time to look further into blood insulin levels and get going on an appropriate wt loss diet. Whether a keto diet is appropriate depends on his individual situation.
