Have a sleep study done. You may find that you have apnea. I am on a CPAP and I am finding that I wake more often during the night but I also sleep more soundly. My waking is because I do toss and turn and am learning to deal with the tubing.
At times before, I would wake like you describe and have trouble going to sleep. Now that is much better.
