You are correct, it is now common to have 4 wellheads on one 12 acre pad. I observed a fracing operation in person 3 weeks ago in the Bakken. 3/4 mile down the road was a drill rig punching in 4 more wells on another 13 acre pad. I do know of one well in the area I was in that had two laterals off one wellhead, one into the Bakken and one into the Threeforks.
