I'll say loose connection as well. All the panels that I see here have copper wire connected to an aluminum bar for ground. I personally don't see anyone use anything on the connections and they rarely give problems.....unless they get loose. It needs to be cleaned well before it goes back together or it will likely give problems again. That melted insulator sounds like something to look closely at too IMO.
