I have used the optometrist at Costco for years. I was not happy once with them and went to another high priced guy and he basically confirmed the first one, I am getting cataracts and just have to live with it at this stage.
The optometrist at Costco and other similar places I have gone are all Optometrist who are licensed to practice in that state. The Costco doc is just a contractor and you have to pay them separately.

On the price of eye glasses, a while back the 60 Minutes program on TV did a story on the price of eye glasses and it turns out that some relatively small company in Italy has almost a world wide monopoly on eye glasses and they have caused the price of glasses to go out of sight. One of the few places that is not under their thumb is Costco. Not sure about WalMart, but Sears is in their web.
