The only power plants that approach 90% efficiency are ones that are able to use waste heat for heating nearby buildings. These are called "combined process" power plants. Very few, if any, grid-scale power plants are able to do this, since they're too far away from large buildings that need heat.

If the waste heat can't be used productively, most fossil-fuel powered plants top out around 60%. Besides that, there are losses involved with moving the electricity around the grid.
