It might also be a strange stress to the gear teeth as well. One might have put more effort into the driving flank geometry, and less into coast down side geometry. That could lead to less life as well, but I am not familiar with industrial gear boxes. Gear design is an area few want to deal with it seems as it can be a bit boring.
