Even with a list forget things at the grocery. Leave clothes in the washer for a couple days. Rarely tell what day it is. BTW stress is pretty high right now. ie; nothing planted, bills piling up, wife works out of town a lot, and young kids to deal with.
I think you have a lot going on in your life from the sounds of it. First get a dr appointment and have your them check you out medically to see if something is out of whack such as blood sugar, blood pressure ect. Then let them see if there is some help they can prescribe. It's good you recognize something isn't right and seeking help before it's too late.
