If I were in your shoes, I wouldn't stop till I saw Dr Ed Law at Steindler Orthopedic Clinic in Iowa City, but I don't know what the distance factor does for you.  Dr Law is a phenominal surgeon and a super nice guy with a bit of a farmer's attitude.  I've entrusted several family members to his care and never regretted a minute of it.
