If you broke the piston, you probably would know it. Usually would see dramatic amounts of blowby gas coming out the crankcase breather tube as well as huge increases in oil consumption.

I have always heard that a worn wrist pin/bushing will slap when you rev the engine.

Might be well worth the time and money to remove the injectors and have them checked. A bad injector can cause some strange noises.

If it is the piston/wrist pin/bushing, it might not happen immediately, but could cost you a lot more if you let it progress to failure. We rebuilt a D333 CAT that had a piston come apart and the rod beat the liner to pieces and then started on the block........... $$$$$$$. Guy driving the old 12E grader must have been deaf. The oil pump had seized up on it and the piston seized first before the rod bearings failed. Oil pump had sucked up a sheet metal locking tab and jammed the gears??????????
