| Some of this I do for a living ,so a few things :
Not saying this is the case , but a lot of people think the hole is drilled larger than the anchor for installation. Not the case .That style anchor needs to have the hole drilled the exact size that the anchor is. Driven in properly they go in tight. A nylon lock nut should not be used on that anchor. They don't come with them. The amount of pressure to get the nylon seated can have an effect on the stud part rotating in the hole before it gets full expansion.
Stainless steel and impacts are never combination because of galling , which is very common on stainless unless it has been coated ( not anti seize) . These anchors would not have been coated. Now throw in the nylock and it can be a bigger problem .
At this point I would use epoxy. You can epoxy the anchor . Let it set and then use a regular nut with a lock washer if you would like. You could also cut threaded rod to the length needed and epoxy it in .
