If I were in your shoes, I would cheat a little from the very beginning. I would cut all the things you mentioned to 5%of what I used to do and go from there.
Seems to me such a dramatic diet change will lead to totally failing and missing out on the benefits of going 95% of the way. I'm no doctor though.
