I've had sinus problems over the years, really bad this last year. You need to forgo your local physician and see a ENT specialist. Then ask them about administering an antibiotic through a nebulizer. The problem is trying to get an antibiotic into sinuses is very hard orally, so best treatment is with oral, and through a nebulizer. They will have to have a pharmacy that does compounding to mix it up for you. I can't remember what my ENT person used but it also had steroids in it I thought. I do know they also gave me a course of oral steroids with it for the first few days. They told me if that doesn't work, then they physically need to go in and do sinus surgery. Basically what that entails is going and scraping the infection out of your sinuses. I haven't needed the surgery to clean mine out, however my father had to have it done this summer, he had a different Doctor than me.

You really need to quit that cheap nose spray you get over the counter, like label says use no more than three days or your body will get addicted to it. It is the same stuff ENT docs use to enlarge your nose and passages to look up your sinuses in check ups. Also from my experience amoxicillin and z pack won't do a thing for you. Never worked for me, maybe once the or twice the Z pack worked but never again. Also an ENT doc needs to check you for polyps in your nose, that could be why you are needing to use nose spray to be able to breath. It will be a rough 24 hours or so coming off the nose spray if you have been on it a long time, trust me I know. Felt like someone shutting off all your air, after that I've been real leery of using the stuff, only used it once or twice in close to 10-12 years.

It was when doc took out my(10-12 years ago) tonsils, adenoids, and polyps in my nose that I finally got feeling better, I was great up until a year or year and a half ago. My primary physician got after me taking daily dose of Claritin-D because she felt it raised my blood pressure. So I quit it but when allergy season hit me, I never could get rid of the crap. I kept listening to her and she kept telling me I had virus, no antibiotics to be used. Well they were wrong, I went though hell, finally went back to ENT doc, he got me on the mend in less than two days.

What I try and use now is Flonase every day for allergies, and ENT doc has a standing prescription for me for his mixture to use in nebulizer if I feel it coming back, so far I haven't needed it since early summer. I'll take any cold, Flu, you name it over sinus infection any day it is rough stuff, I feel for you. But quit messing with reg. docs and see a specialist, I'm betting you will get it cleaned up, good luck.

P.S. It helps me to use saline sinus rinse every day when baling or working in dusty conditions, unbelievable what goes up your nose during the day. I will use it a couple of times a day if needed, along with the Flonase I can handle things pretty good. When you get it cleaned up you will have to find a combination of stuff like Zyrtec or Claritin w/Flonase or Nasacort to keep it from coming back.
