I had one doing the same as #2. If I remember correctly it had something to do with the hyd pressure in the pto valve, not holding it in gear. Can't remember exactly what the fix was. Maybe a relief valve spring, or new seals? Seems like there is also an aluminum piece on the linkage that gets worn or broke.
