And yes, a baby is in ketosis in the womb and in the first week of life.
The question was about breast milk. And yes, breast milk has MCT's and that is delivered via the portal vein to the liver.
But, breast milk is about 50% fat, 40-45% carbohydrate and 6% protein. It's composition varies considerably depending on the mother's diet, the stage of lactation, the gestational age and whether it's the beginning or end of the feeding. Baby's can and do get into a slightly ketotic state if the fat content of the milk goes higher. And yes, breast milk has medium chain triglycerides, but it also has other essential fatty acids as well. To say that a breast fed baby is on a ketogenic diet is not true as there is a good carbohydrate source as well. And even though it is a rich source of MCT's there are other triglycerides as well. To say that a baby is essentially on a ketogenic diet-nature's power fuel (ketones), is an exaggeration and not comparable to an adult on a low carb diet who supplements their diet with MCT oil.
