A lot of "blown head gasket" issues eventually lead to liner removal and block counterbore cutting to get the liner protrusion back up where it needs to be.
The recommendation is to set the liners at .14 when doing an in-frame. 


The actual number is .014", which is at the very top end of Cummins spec of .007 - .014".
