In fairness to the doctors. We as consumers of so-called health care are reliably demanding that only our symptoms be dealt with, and right now.

Our participation in eliminating or at least addressing the root cause is just too much to be bothered with.

Doctors are in many circumstances victims of recent nutrition science history. Patient is obese; ooh, this is going to be brutal, said patient needs to starve himself of fat and calories and run his butt off. Or, what if he figures out insulin and how it interacts with his chosen fuel sources? What about leptin and satiety and the fact that this doesn't need to be torture? Patient is OK with metformin and a statin.

Doctors I have dealt with aren't so much evil + greedy. Just in the dark and they and their customers are exhausted with yoyo diets and bad advice from "trusted sources".
