Ron, I don't have an exact answer for your question - but Kstate has answered for what accounts for the majority of problems. From what I have read, each death or shock is related to different areas and equipment being used over there - all related to improper grounding techniques. Keeping in mine, pending legal action is ongoing, so maybe the exact reason will not be known untill after legal pendings are over? There's no reason this should be happening . . .
