Thanks to Macy, I had a look at the email accounts and there was a second duplicate account that was downloading into Outlook. The genuine account is pop.provider.co.uk and the second account that generated all the spam was pop.provider.co.uk(1).

A couple of odd things about this. Firstly it is not an account that I created. Secondly all the mail in this was spam even though it was configured with my correct email address in it and no genuine mail ever came into it. By selecting actions send/recieve and this individual account it is possible to see what is coming into each account rather than the whole lot in the inbox together. Finally all this spam had incorrect email addresses on it- so how it was getting to me I'm not sure. It's all a.n.other@ my provider.co.uk if you follow.
