Ronm....as you've probably discovered, things like "I want to be cremated and......" need to be in writing somewhere. I've often heard of family disputes over the handling of last rites, etc.

There are a couple of on-line places that you can enter all your wishes. They prompt you to make sure you cover all the bases. Not a legal document, but just your final plans. You include photos you want & write your own epitaph.
