Here in the UK at one time the main Telephone company (B.T.) only enabled exchanges for broadband if the local community guaranteed a minimum number of customers - this meant there was a lot of small scale wireless schemes set up like you're suggesting connected to a satellite link.
Some of these schemes have fallen by the wayside now as all our UK phone exchanges have BB but it gets slow farther out - and some places it still cannot reach.
Here's an example of one such scheme centered on the village of Langtoft.
http://www.langtoft.net/ the fee seems to be about £20 ($30?) a Month for High speed BB which is only a little higher than typical UK cost for those who can get it by landline.
