California/Oregon | Probably really rare. There are several things that could have happened. Driver and idler wheel bolts loosening or idler tires going flat are a couple. Also the air suspension diaphragms can go bad causing the the boogies to lose middle down pressure causing the load and turning pressures to fall onto the driver and idler wheels. During turning there is a tremendous amount of twisting torque on the drivers especially and they will break out and throw the track. Anyway you look at it it sure sucks being that guy.
