We just finished an addition with crawlspace at my Mom and Dad's and used the ICF forms for the walls. Just used 1" base for the floor. Don't see much sense in pouring a concrete floor in an unused space. There is a sprayfoam kit there to do the sill boards, but that isn't completed yet. The crawlspace is nice and warm, and don't notice the floors being cold.
