Leesburg, Ohio
All the above.

Based on university and private agronomist recs.
pH is primary, but mag:cal ratio should be something like 1:5 or 1:6

For decades, most around here used high mag lime, for two reasons: 1) livestock needed more mag, especially on new green growth in the spring, to prevent grass tetany, and 2) most quarries in the the area had very high quality high mag lime readily available. High cal was harder to find.
So, our mag levels were in the 20's, and cal was in the 40's, so more like 1:2. That limits uptake of some nutrients.
The last 20 years, we have been using high mag lime exclusively, and our balance is improving.
Other areas may have different scenarios, that's why I suggested paying attention to your test levels on that ratio when determining which type of lime to buy...some areas may be in good balance, some not.
It's all a balancing act, right?
