South Central MN | Funny story. Used to work for one of the major ag equipment manufacturers. We kept getting warranty claims on a certain bolt backing out. Went to the line to investigate the problem, they were hand-tapping this particular hole on the assembly line. They'd welded a hex shank onto a tap, and then they ran the tap with an impact driver. Buzzed it in and out and done. problem was the homemade tap wasn't square at all so it wobbled, plus they were apparently hard to make because this particular tap was missing about half its teeth and they hadn't replaced it. It is the most abused tap I've ever seen. I'd wager the OP's tap once he gets it out will be in better shape than this one* We took that tap away and I believe the design got changed with the next release to allow for tapping at a milling station instead.
