Subject: Why is this Dayton wheel spinning on wedges?

North Central Alabama
I have a 88 GMC 7000 dump truck that i use aroung the farm.I noticed this morning the rim had sliped on the wedges as you can see in the picture.I jacked the truck up and loosened all the wedges and spung rim back around and thightened the heck out of the wedges with a 3/4" impact.I used the truck today and it sliped again.
The rim does not have knots or they are worn off on it at the valve stem.As you can see the metal valve stem is in danger of being sheared off. What could be wrong?
