We did as you described. We used Forma-drain for the footing forms and I had a 6 inch rock base under the slab so i was kind of setup from the start. When I drilled the hole I just made sure I was over the top of the Forma-drain. I did the install about 5 years ago the fan has been running ever since. Our levels were near 30 and are now acceptable. I used 4 inch PVC.
