Drain plumbing question, Master Plumbers?

Subject: Drain plumbing question, Master Plumbers?



SW Ohio
Doing some work on Mitchco's house, replacing all the drains from septic tank back to everything, John, shower, bathsink, kitchen, washing machine.
Have read many conflicting "expert opinions" on how to do it, ready for the NAT version
First, what is the recommended fall for the main line? Straight shot with no turns using 4" PVC, so far have it set at 1/4" fall per foot or 2%

Next, the first connection from the tank will be 4x4x3 "sweep tee" to the john which is about 20" above the main line at this point, and maybe two foot sideways, should the "sweep tee" be pointed to the side or the top?
Same thing for the shower, should the tee be pointed sideways or up?
same for the washer and kitchen?
Where should the vent go? last in line away from tank, first, or anywhere?
This all in a crawl space with very limited room, we have had to dig our way to get to the old pipe and dig deeper to set the new one about ten inches lower
