It messes with everything, just holding the gun steady is off (backwards) as your muscles are trained for normal gravity, recoil is worse - again gravity, aiming point is affected. Target location also "changes" - opposite sides when upside down, it wouldn't matter quite as much if you werent swinging from left to right during the rotation, but that's enough to give a slight disorientation
