Where did it break? Just the head break off? I've seen them break from a washer being put on upside down. A flat washer has a sharp edge and a rounded edge. The rounded edge always needs to be against the head of the bolt. If the sharp edge is against the head of the bolt, it can mark it, even in just one spot, and cause a stress crack to start.
