My first thought is that the regulator is stuck, but that doesn’t explain oil fill cap blowing off. Maybe excess oil pressure causes crankcase pressure in crankcase and blew oil cap off. I would screw in a mechanical gauge and check actual oil pressure. Is engine missing on any cylinders? Any blowby ?
