Make sure the SAM-200 is on version 1.04. 1.02 would kick up that fault. If it is on 1.04 it is possible you need to replace the SAM-200.
Fault 12 is for a motor/encoder mismatch. It means the stepper motor in the SAM is not moving as much as it is supposed to and the encoder that counts where the motor is kicks in the fault.
