south east SD		2290.gov can't remember if that is all that is for sure?

Fill it out, print it off, put in a envelope, stamp, put it in my box and wait a couple weeks for it to be returned stamped.

Did mine for 2018 in August 2017. Get my plates in May and my the form is in my file waiting to be used.
