hmm don't really know what to tell you. Did you try a blank project to see what comes off the card. I do that sometimes to just to double check if it looks like something is missing. Only thing I can think of many should have been a clean card. Could look to see and make sure the date is right when you assign a new task for the new task name just a thought.
