Phil's book mentioned above shows a lot of different causes of inconsistent wheat. I believe the problem is if it reaches flag leaf or bloom etc at different times for whatever reason. Impossible to time fungicide applications correctly for the whole field for example. Like Matt mentions, doubling up/overlap on end rows etc can result in lodging.
