I'm pretty sure we pulled our boiler-plate lease form off the net somewhere.  Try doing a search at iastate.edu..............seems like it was in the same general area as the custom rate listing.  We just made a few minor changes to it, didn't affect the meanings at all, just made it fit our operation a bit better.  Easy-peasy!
