I'm with Lowell.................if I were in that spot, I'd choke 'em in heel dust heading to Mayo or MD Anderson.  And I don't mean Mayo in AZ.....Mayo in Rochester MN.   Any local physician worth their salt will give a referral to either one, if insurance requires a referral.
