You should at minimum replace them in pairs (one inside one outside). The new pair should be either the front set or the rear set. The theory is that since the new wheels are taller, and the 45's have a solid undercarriage, one new wheel will carry more than its share of the load. For whatever its worth, the only problem i've seen with just replacing one is that it wears out quicker.
