I just put one on our combine today, thanks, i'll double check it. I was taking my time putting it on, kinda was thinking something like what you just described could happen. The retaining washer has one leg turned down into a hole in the bushing, the others turn up to secure the nut. I could see how the leg that turns down could possibly be damaged while tightening the nut, allowing the nut to work itself free over time.
