| Do you have MS Word? If so, you should be able to find a template on there. I looked on my copy and it is there.
Click 'Open|New'. 'New From Template|general Template'. You should be able to find it there.
If you don't have MS Word, maybe you can post what Word Processing program you are using. I would think any word processing program would have a similar template.
I am sure you can find other options, but that is a place to start, and it gives you a general idea what you are looking for.
