NW Ohio
Are you wanting this in software or just jotting down numbers to see the fall?

If software, use WM Survey.

If all you need is elevation, change the settings so Altitude is displayed on the monitor, stop your vehicle for a minute and jot down the Alt. Do this on all the highs and lows of the run, very similar to a laser ground survey. Requires some math for fall, rise divided by run = grade so 1ft rise in 1000 feet = 0.001, move the decimal point two positions to the right equals a 0.1% grade. If working with length numbers with the last two digits being 00, just drop them in the division, so 1 divided by 10 = 0.1 (1 tenth grade).
