My wife is a nurse, and her hands were always cracking and painful from washing them so much for sanitary reasons. She started using a product called working hands about five-six years ago and swears by it. She puts it on her hands every night before bed. It comes in a green tub about four inches across and an inch high. We find it at farm and fleet, sometimes near the animal care products.
