Twist the screw as you apply pressure upward on the lens. It helps if you can get one screw out. The insert will pull itself out of the lens and you can get it out. Then use a vice grip on the insert and twist the screw out maybe, might end up cutting it off. I got by a few years just pressing the lens back down on the insert and tightening the other screw
